For my design, I used a reset-ic to source the RC (instead of VCC)... it looked like you had a similar intent.
Since your reset signal is directly on the pin, may want to ensure that it is super clean.

Should be clean due to the RC filter but I will check it (I have another board coming with a better layout, as I suspect the problem is eddie currents in the groundplane).

I'm also experiencing the same problem. I'm using esp32 pico d4 in a custom PCB I'm using U0TXD and U0RXD pins of UART to flash it.

I'm getting this error in a loop

i checked my power supply its getting solid 3.3v to the power pins also it can go up to 1.5A
